Meadowood Health Pavilion Admission Policy

It is the policy of the Meadowood Health Pavilion to admit and to treat all residents without regard to race, color, religion, national origin, handicap, or age. You need not be affiliated with Meadowood or Indiana University to be admitted.

The same policy is applied within the Meadowood Health Pavilion for room assignments.

All facilities and services are made available to residents, visitors, and guests without regard to race, color, religion, national origin, handicap, or age.

All persons and organizations having occasion either to refer residents for services or to recommend the Meadowood Health Pavilion are advised to do so without regard to the resident's race, color, religion, national origin, or age.

The person designated to coordinate compliance with Section 504 of the Rehabilitation Act of 1973 (nondiscrimination against the handicapped) is Susan Bookout who can be reached at (812) 336-7060.

Meadowood Health Pavilion does not require a third party guarantee of payment to the facility as a condition of admission, or continued stay in the facility. An individual who has legal access to a resident's income or resources available to pay for facility care is required to sign a contract, without incurring personal financial liability, to provide facility payment from the resident's income or resources.

Meadowood Health Pavilion does not require residents or potential residents to waive their rights to Medicare or Medicaid. No oral or written assurances are required that residents or potential residents are not eligible for, or will not apply for, Medicare or Medicaid benefits. Meadowood Health Pavilion does not participate in the Medicaid program. In order to receive Medicaid benefits, that resident would need to transfer to a Medicaid-participating facility.
