Meadowood Health Pavilion Admission Requirements

 

Meadowood conducts a pre-admission screening process with any resident considering short- or long-term placement in the facility. When a resident is contemplating placement in the facility from a home environment, a pre-admission evaluation will be conducted in the home setting by the appropriate Meadowood staff. Any resident entering the Health Pavilion must fulfill the following state requirements governing health care facilities:

  • Meadowood must receive a complete history and physical examination signed by resident's physician on or prior to day of admission.
  • Meadowood must receive a resident's written chest x-ray report that has been taken within the past six months.
  • Resident's physician must provide written orders for admission to facility.
  • Admission paperwork must be completed through a scheduled appointment with the Assistant Administrator or Health Pavilion Social Worker.
  • Pre-admission screening process must be completed by an Area 10 Agency on Aging Case Manager (telephone number: 812-876-3383).

A resident must be admitted under the care of a physician of his/her choice who is licensed to practice medicine in the State of Indiana. The physician must be willing to make scheduled facility visits at least every thirty days for the first 90 days after admission and at least every 60 days thereafter.
